Weeks Medical Center issued the following announcement on March.

We’re looking for your feedback! A Community Health Needs Assessment (CHNA) will help the providers in the North Country region determine the health needs of our community.

This survey should take less than 15 minutes and will be open for three weeks, although we look forward to your response as quickly as possible. Your time and effort are greatly appreciated!
